You can try Darul Qurra, a Birmingham based organisation, run by Qari abu Adam Akhlaq al-Azhari, Mikhaael Mala et al. It offers advanced qir'at and tajweed courses (also online classes) and has earned very good reviews.
I haven't tried it myself but came across Qari abu Adam this week and was impressed by his dedication and sincere approach.

With respect to receiving ijazah to teach, how long does the process take in general?

In the west, dawat e Islami run good classes on tajweed. They're the only sunni organisation I know that I can trust on that point, largely.

You can also use online tutors. For example, I have a tajweed tutor on italki which is a language learning website but a lot of the Arabic teachers there (mostly Egyptian) teach tajweed as a separate subject as well.

If one wishes their child to learn to recite the Qur'an beautifully with correct tajweed, to learn the different qira'ah, to receive ijazah to teach, etc.:

    - what are some good institutions in Muslim lands that one can go for this purpose?
    - what are some good institutions or teachers in the West for this purpose?
    - how long is the process in general?
    - does anyone have personal experiences they can share?
